Case Management Coordination...
HMS has directed case management coordination and capacity-building assistance for hundreds of minority community-based organizations. Through funding provided by the Centers for Disease Control and Prevention’s (2002-2005), HMS was responsible for coordinating and delivering developmentally appropriate, linguistically specific and culturally appropriate individualized technical assistance for African American, Hispanic/Latino, Asian American Pacific Islander and Native American community-based organizations.
As a capacity-building technical assistance provider, under contract to the National Minority AIDS Council, HMS created a specialized action plan for each organization. Assistance was offered in a variety of areas, including:
